Is there a form that you suggest to use for collecting our data sample?  Use the charts in Tier for fast?  Or type up in excel document.  I like the ones that were used as an example in folders and powerpoint.  I would think each grade level would need different information.  Like kindergarten would have letter sounds, first grade nonsense words, and upper grades word read correct with accuracy and comprehension score.  Would anyone be willing to share what their school uses for this part?  Thanks so much!
